Eli Lilly (NYSE:LLY) and Pfizer (NYSE:PFE) just gave investors two very different pharma stories. Lilly’s latest quarter showcased runaway incretin demand and a raised outlook. Pfizer’s showed a diversified book slowly stabilizing after COVID, with a fresh obesity bet stapled on. Heading into 2027, the question is whether uncapped growth or defensive transformation deserves your capital.
Mounjaro Runs the Table, Pfizer Steadies the Base
Lilly’s Q2 2026 earnings report was category-defining. Revenue hit $22.97B, up 47.7% year over year, driven by Mounjaro at $9.94B (+91%) and Zepbound at $4.93B (+46%). International Mounjaro sales jumped 172% following China NRDL listing, a real signal that GLP-1 demand is global. CEO David Ricks called Lilly’s future “never been brighter” after 150 years, and management raised FY2026 revenue guidance to $85.0B to $87.0B.
Pfizer’s Q1 2026 was quieter but more encouraging than skeptics expected. Revenue reached $14.45B, up 5.4%, with adjusted EPS of $0.75. Growth came from newer assets: Padcev at $591M (+39%), Nurtec at $353M (+42%), and oncology biosimilars at $409M (+55%). COVID drag continued, with Comirnaty down 59% and Paxlovid down 62%. Albert Bourla told investors Pfizer will “successfully navigate this defining period”, leaning on oncology and a late obesity push.
