An AI analysis of more than 400,000 Reddit posts surfaced side effects that clinical trials never captured.

Here are the common, well known side effects of GLP-1 drugs like Ozempic, Mounjaro, Wegovy and Zepbound: nausea, vomiting, fatigue, constipation and diarrhea. GLP-1 drugs sometimes trigger these effects because they slow digestion, giving the sensation of feeling full, and occasionally confusing the intestines into speeding up transit. Fatigue can come from reduced caloric intake and dehydration.

These effects are printed on every GLP-1 label and are a focus of doctor-patient discussions when prescribing. But a growing body of evidence suggests the label may be incomplete. In my experience, some patients mention a second set of symptoms that doesn’t appear in the label.

In April 2026, a study in Nature Health assessed GLP-1 side effects reported online. The team used AI to analyze 410,198 Reddit posts from May 2019 and June 2025. Among the 67,008 people who identified themselves as semaglutide or tirzepatide users, 44% described at least one side effect and averaged 2.7 symptoms.

Familiar complaints dominated, suggesting that the signals in the trial data largely match the Reddit reports. This included nausea (37%), fatigue (17%), vomiting (16%), constipation (15%) and diarrhea (13%). However, there were several other categories of effects that are less often discussed and may be just as debilitating.

Here are some of the less common GLP-1 side effects people should consider before starting treatment.

1. Menstrual changes and the “Ozempic baby” phenomenon

Reproductive symptoms were reported by 3.8% of users: inter-menstrual bleeding (0.9%), heavy bleeding (0.9%), irregular menstruation (0.7%), menstrual disorder (0.6%) and painful periods (0.6%). According to the authors: “…these rates would be higher in female-only samples.”

Simultaneously, fertility clinics have reported the opposite phenomenon: women with longstanding infertility or polycystic ovary syndrome suddenly ovulating.

Here’s why. Adipose tissue converts androgens into estrogen, so rapid fat loss shifts circulating estrogen quickly. This can impact the cycle’s rhythm before the system re-equilibrates. GLP-1 drugs also act on hypothalamic receptors in the brain to suppress appetite, and the hypothalamus is also central to menstrual regulation.

In polycystic ovarian syndrome (PCOS), the reproductive effect can be therapeutic. In a randomized trial of 100 overweight and obese women with PCOS and irregular menstrual cycles, adding semaglutide to metformin for 16 weeks restored their cycles in 73% of participants, compared with 42% on only metformin. Natural pregnancy rates over the following months were also higher: 35% versus 15%.

This pregnancy risk is a phenomenon called “Ozempic baby:” restored fertility in a woman who was previously unable to conceive. Another issue is the drug interaction. Tirzepatide’s delayed gastric emptying can reduce the efficacy of oral contraceptives,. The label advises switching to a non-oral method or adding a barrier method for four weeks after starting and after each dose increase. Semaglutide has not shown a clinically meaningful effect on oral contraceptive levels.

This is important because GLP-1s are not recommended in pregnancy. Animal studies show fetal harm. And while a prospective cohort of 168 first-trimester exposures found no increased risk of major birth defects or pregnancy loss, the sample is too small to be reassuring. Semaglutide’s label instructs discontinuation at least two months before a planned pregnancy.

What can help: Women of reproductive age starting tirzepatide should use a backup or non-oral contraceptive method during initiation and dose escalations in consultation with a doctor. Anyone with PCOS or prior infertility should assume fertility may return and plan accordingly. Finally, heavy or prolonged bleeding which should prompt a physician evaluation.

2. Taste, smell, shifting “food noise” and sulfur belches

Reddit users described food tasting differently as well as heightened sensitivity to certain odors, a metallic taste, or sulfur-smelling belches. In the analysis, 1.4% reported an altered sense of taste and 0.6% a broader taste disorder. Burping was far more common at 6.9%, with 0.6% reporting a consistent breath odor.

Semaglutide physically changes the experience of food by altering the tongue’s taste sensitivity, changing how taste genes work and shifting the brain’s reaction to sweets.

Naturally, taste buds produce GLP-1 which talk to nearby nerves that handle sweet flavors. Semaglutide can hook into this system, blunting the usual pleasure from eating high-fat and sugary treats. The combined effect on the tongue and the brain’s reward center is one reason why the medicine can successfully silences constant food cravings.

Sulfur burps are a separate, mechanical problem: slowed gastric emptying allows food to sit and ferment, generating hydrogen sulfide.

What can help: Taste changes typically ease as the body adjusts and often flare after dose increases. For sulfur burps, consider limiting sulfur-rich and high-fat foods such as eggs, garlic, onions, cruciferous vegetables, fried food, and instead eating smaller and more frequent meals. Also avoid carbonation, and considering use of simethicone (Gas-X) or bismuth subsalicylate (Pepto-Bismol) under the direction of a doctor. Importantly, sulfur burps that are paired with vomiting may signal delayed gastric emptying should be evaluated by a physician.

3. Temperature disruption: chills and hot flashes

Some patients taking GLP-1s describe sudden shivering fits, an inability to warm up, or the reverse — abrupt flushing and heat. Reddit users reported such symptoms in a small proportion of cases: chills were in 1.1% of users reporting side effects, excessive sweating in 1.2%, feeling cold in 0.6%, fever in 0.5% and hot flushes 0.5%. However, tirzepatide users reported temperature-related symptoms at higher rates between 1% and 4% of the time.

There is a biological rationale for this in animal studies. GLP-1 receptors are densely expressed in the hypothalamus, the brain’s thermostat. Research shows that GLP-1 weight-loss signals can directly force the body to burn fat for heat, entirely independent of how much food is consumed.

Specifically, stimulating GLP-1 targets in one area of the hypothalamus triggers energy-burning “brown fat” and converts standard white fat into a more active, heat-producing state, while blocking these signals in a neighboring brain region shuts the heating process down. Rapid loss of insulating subcutaneous fat compounds the issue, as does the reduced caloric intake that lowers the body’s baseline metabolic heat production.

What can help: Clinicians generally recommend slower dose escalation, since temperature complaints tend to cluster around titration steps. Adequate caloric and fluid intake matters as undereating can amplify cold intolerance. However, persistent chills warrant a medical work-up for iron deficiency, B12 deficiency and thyroid dysfunction under the advice of a doctor.

4. Hair loss

Hair loss was reported by 3.1% of Reddit users describing side effects. Rapid weight loss is a physiological stressor that can push hair follicles prematurely out of their growth phase and into a resting phase. Shedding typically begins around three months after the weight loss starts. The pattern also showed up in the semaglutide trials, where hair loss was reported by roughly 3% of participants versus about 1% on placebo.

What can help: GLP-1-related hair loss is often self-limited and generally resolves within six to nine months once weight stabilizes. Slower weight loss can reduce the physiological stress that triggers it. Adequate also protein matters, as do iron, zinc and vitamin D, which are commonly depleted when caloric intake drops sharply and which independently can lead to shedding. Patchy or persistent loss is a different problem and warrants evaluation by a dermatologist.

5. “Ozempic face” and “Ozempic vulva”: Structural volume loss in the face and genital tissue

Here’s the issue: the body doesn’t choose where it burns fat. A 2025 retrospective imaging cohort of 20 GLP-1 users with before-and-after scans found a 9% loss of midface volume, roughly 7% per 10 kilograms lost. And superficial fat compartments deplete faster than deep ones. The facial fat pads that create fullness in the cheeks and temples sometimes shrink faster than skin can retract, producing a characteristic hollowing and laxity.

The same physics applies elsewhere. Volume loss in the genital area can also occur in the mons pubis and labia majora as subcutaneous fat recedes with treatment.

What can help: Pace is the single biggest lever to reduce the chance of an “Ozempic face” or an “Ozempic vulva.” Slower weight loss can give the skin time to accommodate. Maintaining protein intake and resistance training can also preserve the underlying structure. Dermatologists can also offer fillers, injectables and energy-based tightening for facial volume.

Importantly, none of this argues against a drug class that has meaningfully changed treatment for obesity and type 2 diabetes. But it does argue for patients to be better informed about potential side effects. Clinical trials measure what they set out to measure. They won’t necessarily surface every symptom that no one thought to ask about.

The value of the Nature Health study is not to suggest that Reddit is a reliable clinical instrument. The study percentages cannot establish either true frequency or causality. But it does capture what patients report in the real world.

Finally, many of these effects are manageable, and several respond to the same interventions: slower titration, adequate protein, deliberate hydration, resistance training and a prescriber willing to hear about a symptom that isn’t on the label.
